Application

To apply to TUSOM you will need to fill out the PDF application either online or click the Print Form button and fill out by typing or printing neatly.

Please complete the entire form then sign and mail along with the following items:

• A passport photo of yourself
• Transcripts from each college or university that you have attended. Student (unofficial) copies are acceptable for the admissions process.
• A check for $75.00 in U.S. denominated funds - the non-refundable application fee made payable to: Trinity School of Medicine.
• Letters of Recommendation: Please have at least two letters of recommendation or a letter from the pre-health advisory committee (if applicable) sent directly to Trinity School of Medicine as soon as possible. We can begin processing your application without them but they are required to complete the admission process.
