Application for Admission

To apply to Trinity you will need to fill out the PDF application either online or click the Print Form button and fill out by typing or printing neatly.

Please complete the entire form then sign and mail along with the following items:

  • Transcripts from each college or university that you have attended. Student (unofficial) copies are acceptable for the admissions process.
  • Application fee in the amount of $75.00 US: Made payable to Trinity School of Medicine. The application fee is non-refundable.
  • Letters of Recommendation: Please have at least two letters of recommendation or a letter from the pre-health advisory committee (if applicable) sent directly to Trinity School of Medicine as soon as possible. We can begin processing your application without them but they are required to complete the admission process.
  • A passport photo.
